Work Permits
Instructions for completing a work permit application:
*You are required to have a physical performed, or have one on file from a sport dated within one calendar year in order for your permit application to be processed.
Fill out the top section and have your parent/guardian sign it. You are required to get or have a physical on file from your doctor's office, Minute Clinic, or Urgent Care. The doctor needs to sign the back of the application and have the office stamp it with their office information.
*If we have a current physical on file with the athletic or ROTC program, your parent/guardian can sign the form and a new physical will not be required. Physicals are valid for one year from the date of the exam.
Take the form to your new employer and they will sign, date, and fill in the information requested.
Scan and email the completed application to britt.kline@swcsd.us and the information will be entered on the State of Ohio website. You will then have the permit emailed back to you. You will need to print, sign, and give it to your employer. You may also bring in the completed form to the guidance office and have the work permit issued then.
